All players must be members of USA Hockey to participate in any of our programs. Please visit www.usahockeyregistration.com to complete this process and obtain a membership number. You will need the player's membership number to complete your registration in one of our programs.
A separate registration must be filled out for each player/program.
If you are registering a child younger than 18 years of age, please be sure that you are registering your child and not yourself.
Tryout fees are due at the time of registration. You will be prompted for on-line payment at the completion of this form. Lawrence Flames accepts Visa, Mastercard, Discover & American Express. If you do not pay the registration fee, your registration will be not be accepted.
